Acceptance of applications by HCI-Colombo does not guarantee the issuance of Passports & Passport related services. Mission reserves the right to decline an application in case it deems appropriate to do so without giving any reasoning. The copies of all documents issued by authorities/institutions/organizations of Sri Lanka which needs to be submitted to the High Commission of India, Colombo for availing passport services, should henceforth be attested by the Ministry of Foreign Affairs, Sri Lanka.
This includes, by way of example, Marriage Certificate, Divorce Certificate, House Registration Certificate, Death Certificate, Birth Certificate, Certificate of Name Change, etc.
Note: Before applying for the fresh passport, the birth of the minor should be registered with the High Commission of India Colombo to obtain the Indian Birth Certificate.

Minor (Fresh)

1. Printout of online submitted application with Photograph and Signature on Application form along with 2 extra Photographs (Application signed by the child/ child’s thumb impression (Left thumb only) applicant where needed).
2. Photograph should be taken as per guidelines for ICAO Complaint:

3. Copy of Indian Birth Certificate
4. If the baby is born in Sri Lanka, the Attested Sri Lankan Birth Certificate, Original, and Copy of the English Translation have to be submitted.
5. No objection letter from Parents
• If parents or one of the parents live in abroad should be submit living country’s Indian High Commission attested No objection letter
6. Passport copies of parents
7. Marriage certificate copy of parents
8. Copy of the parent’s resident visa
9. If Indian parents work in Sri Lanka
• Should give an Employment confirmation letter from the Company
10. Address proofs to be submitted – At Least 2 of the following documents have to be submitted.
• As Indian Address Proof – (Aadhar card, Election Card, Water bill, Electricity bill, Telephone bill, Income – Tax card, driving license or copy of rent Agreement)
• As Sri Lankan Address Proof- (Water bill, Electricity bill, Telephone bill, Driving License, Copy of the Bank A/C book)

Note:

• If need change your address in current passport
• Applicant has to select ‘Change of Applicant details-Address’- at the time of filling the online application & the reason for the renewal should be mentioned in the Applicant’s request letter.
• Address proofs to be submitted – At Least 2 of the following documents has to be submitted.
• As Indian Address Proof – (Aadhar card, Election Card, Water bill, Electricity bill, Telephone bill, Income – Tax card, driving license or copy of rent Agreement)
• As Sri Lankan Address Proof- (Water bill, Electricity bill, Telephone bill, Driving License, Copy of the Bank A/C book)
• If need change your Name in current passport
• Applicant has to select ‘Change of Applicant details-Address’- at the time of filling the online application & the reason for the renewal should be mentioned in the Applicant’s request letter
• Affidavit Form to be filled by applicant in case change of name for women after divorce
• One of the following documents has to be submitted
• Copy of the birth certificate
• Copy of the education certificate
• If the passport is lost
• Covering letter mentioning details of how the passport was lost.
• Police Report (English Translated)
• If passport is Damaged
• Covering letter mentioning details how the passport was damaged.
• Original damaged Passport. Original
• All the supporting documents should be in English
• All the applicants should be able to produce the original supporting documents if needed to Mission at the time of submission. (Only copies of the original documents will be accepted with the passport renewal application.)
• The current passport will be automatically cancelled after the payment is Done.
